Re-issuance of the Certificate of cooperative alliance registration (if lost)

a) Implementation order:

- Step 1: When the registration certificate is lost, the cooperative alliance shall submit documents to request for re-issuance of the registration certificate to the cooperative alliance registration agency.

- Step 2: Upon receiving the documents, the cooperative alliance registration agency:

+ Request the applicant to submit a valid copy of valid identity card or passport;

+ In case of being authorized, it is required to prepare the following papers: The service supply contract between the person establishing the cooperative alliance, the cooperative alliance and the organization providing services of submitting documents and receiving results; the recommendation letter of that organization for the individual to submit documents and receive results; or the authorization letter the individual to submit documents and receive results in accordance with law.

If documents are valid, the cooperative alliance registration agency shall issue a receipt to the cooperative alliance.

- Step 3: The registration agency shall re-issue the cooperative alliance registration certificate. In case of not re-issuing the registration certificate, the cooperative alliance registration agency shall send a written notice stating the reasons for the cooperative alliance to know.

In accordance with the provisions of Clause 6, Article 2 and subparagraph b, Clause 2, Article 3 in Circular No. 02/2014/TT-BTC dated 02/01/2014 of the Ministry of Finance guiding charges and fees to be decided by people's Councils of provinces and Centrally-run cities.
