Re-issuance of the Certificate of cooperative alliance registration (if change from Business registration certificate to Certificate of cooperative alliance registration)

a) Implementation order:

- Step 1: If the cooperative alliance issued the business registration certificate before August 1^st, 2014 wants to change from Business registration certificate to Certificate of cooperative alliance registration without changing the contents of cooperative alliance registration, it shall submit the request for change to the cooperative alliance registration agency.

- Step 2: Upon receiving the documents, the cooperative alliance registration agency:

+ Request the applicant to submit a valid copy of valid identity card or passport;

+ In case of being authorized, it is required to prepare the following papers: The service supply contract between the person establishing the cooperative alliance, the cooperative alliance and the organization providing services of submitting documents and receiving results; the recommendation letter of that organization for the individual to submit documents and receive results; or the authorization letter the individual to submit documents and receive results in accordance with law.

If documents are valid, the cooperative alliance registration agency shall issue a receipt to the cooperative alliance.

- Step 3: The registration agency shall issue the certificate.

- Step 4: The cooperative alliance shall return the original of Business registration certificate previously issued upon being issued the new one.

In accordance with the provisions of Clause 6, Article 2 and subparagraph b, Clause 2, Article 3 in Circular No. 02/2014/TT-BTC dated 02/01/2014 of the Ministry of Finance guiding charges and fees to be decided by people's Councils of provinces and Centrally-run cities.
